Changes necessary to support SEPARATE_NOBITS_REGION feature ...

Since BL31 PROGBITS and BL31 NOBITS sections are going to be
in non-adjacent memory regions, potentially far from each other,
some fixes are needed to support it completely.

1. adr instruction only allows computing the effective address
of a location only within 1MB range of the PC. However, adrp
instruction together with an add permits position independent
address of any location with 4GB range of PC.

2. Since BL31 _RW_END_ marks the end of BL31 image, care must be
taken that it is aligned to page size since we map this memory
region in BL31 using xlat_v2 lib utils which mandate alignment of
image size to page granularity.

pmf: Make the runtime instrumentation work on AArch32 ...

Ported the pmf asm macros and the asm code in the bl31 entrypoint
necessary for the instrumentation to AArch32.

Since smc dispatch is handled by the bl32 payload on AArch32, we
provide this service only if AARCH32_SP=sp_min is set.

PIE: make call to GDT relocation fixup generalized ...

When a Firmware is complied as Position Independent Executable it needs
to request GDT fixup by passing size of the memory region to
el3_entrypoint_common macro.
The Global descriptor table fixup will be done early on during cold boot
process of primary core.

Currently only BL31 supports PIE, but in future when BL2_AT_EL3 will be
compiled as PIE, it can simply pass fixup size to the common el3
entrypoint macro to fixup GDT.

The reason for this patch was to overcome the bug introduced by SHA
330ead806 which called fixup routine for each core causing
re-initializing of global pointers thus overwriting any changes
done by the previous core.

Refactor ARMv8.3 Pointer Authentication support code ...

This patch provides the following features and makes modifications
listed below:
- Individual APIAKey key generation for each CPU.
- New key generation on every BL31 warm boot and TSP CPU On event.
- Per-CPU storage of APIAKey added in percpu_data[]
  of cpu_data structure.
- `plat_init_apiakey()` function replaced with `plat_init_apkey()`
  which returns 128-bit value and uses Generic timer physical counter
  value to increase the randomness of the generated key.
  The new function can be used for generation of all ARMv8.3-PAuth keys
- ARMv8.3-PAuth specific code placed in `lib\extensions\pauth`.
- New `pauth_init_enable_el1()` and `pauth_init_enable_el3()` functions
  generate, program and enable APIAKey_EL1 for EL1 and EL3 respectively;
  pauth_disable_el1()` and `pauth_disable_el3()` functions disable
  PAuth for EL1 and EL3 respectively;
  `pauth_load_bl31_apiakey()` loads saved per-CPU APIAKey_EL1 from
  cpu-data structure.
- Combined `save_gp_pauth_registers()` function replaces calls to
  `save_gp_registers()` and `pauth_context_save()`;
  `restore_gp_pauth_registers()` replaces `pauth_context_restore()`
  and `restore_gp_registers()` calls.
- `restore_gp_registers_eret()` function removed with corresponding
  code placed in `el3_exit()`.
- Fixed the issue when `pauth_t pauth_ctx` structure allocated space
  for 12 uint64_t PAuth registers instead of 10 by removal of macro
  CTX_PACGAKEY_END from `include/lib/el3_runtime/aarch64/context.h`
  and assigning its value to CTX_PAUTH_REGS_END.
- Use of MODE_SP_ELX and MODE_SP_EL0 macro definitions
  in `msr	spsel`  instruction instead of hard-coded values.
- Changes in documentation related to ARMv8.3-PAuth and ARMv8.5-BTI.

Add support for Branch Target Identification ...

This patch adds the functionality needed for platforms to provide
Branch Target Identification (BTI) extension, introduced to AArch64
in Armv8.5-A by adding BTI instruction used to mark valid targets
for indirect branches. The patch sets new GP bit [50] to the stage 1
Translation Table Block and Page entries to denote guarded EL3 code
pages which will cause processor to trap instructions in protected
pages trying to perform an indirect branch to any instruction other
than BTI.
BTI feature is selected by BRANCH_PROTECTION option which supersedes
the previous ENABLE_PAUTH used for Armv8.3-A Pointer Authentication
and is disabled by default. Enabling BTI requires compiler support
and was tested with GCC versions 9.0.0, 9.0.1 and 10.0.0.
The assembly macros and helpers are modified to accommodate the BTI
instruction.
This is an experimental feature.
Note. The previous ENABLE_PAUTH build option to enable PAuth in EL3
is now made as an internal flag and BRANCH_PROTECTION flag should be
used instead to enable Pointer Authentication.
Note. USE_LIBROM=1 option is currently not supported.

PIE: Fix reloc at the beginning of bl31 entrypoint ...

The relocation fixup code must be called at the beginning of bl31
entrypoint to ensure that CPU specific reset handlers are fixed up for
relocations.

BL31: Enable pointer authentication support in warm boot path ...

In the current Pointer Authentication support added in
commit b86048c
PAuth gets enabled in BL31 cold boot entrypoint only,
(see bl31_entrypoint() in bl31\aarch64\bl31_entrypoint.S)
but not in bl31_warm_entrypoint().
This results in EnIA bit [31] in SCTLR_EL3 not being set
and pointer authentication disabled after CPU wake-up event.

Sanitise includes across codebase ...

Enforce full include path for includes. Deprecate old paths.

The following folders inside include/lib have been left unchanged:

- include/lib/cpus/${ARCH}
- include/lib/el3_runtime/${ARCH}

The reason for this change is that having a global namespace for
includes isn't a good idea. It defeats one of the advantages of having
folders and it introduces problems that are sometimes subtle (because
you may not know the header you are actually including if there are two
of them).

For example, this patch had to be created because two headers were
called the same way: e0ea0928d5b7 ("Fix gpio includes of mt8173 platform
to avoid collision."). More recently, this patch has had similar
problems: 46f9b2c3a282 ("drivers: add tzc380 support").

This problem was introduced in commit 4ecca33988b9 ("Move include and
source files to logical locations"). At that time, there weren't too
many headers so it wasn't a real issue. However, time has shown that
this creates problems.

Platforms that want to preserve the way they include headers may add the
removed paths to PLAT_INCLUDES, but this is discouraged.

PIE: Position Independant Executable support for BL31 ...

This patch introduces Position Independant Executable(PIE) support
in TF-A. As a initial prototype, only BL31 can support PIE. A trivial
dynamic linker is implemented which supports fixing up Global Offset
Table(GOT) and Dynamic relocations(.rela.dyn). The fixup_gdt_reloc()
helper function implements this linker and this needs to be called
early in the boot sequence prior to invoking C functions. The GOT is
placed in the RO section of BL31 binary for improved security and the
BL31 linker script is modified to export the appropriate symbols
required for the dynamic linker.

The C compiler always generates PC relative addresses to linker symbols
and hence referencing symbols exporting constants are a problem when
relocating the binary. Hence the reference to the
`__PERCPU_TIMESTAMP_SIZE__` symbol in PMF is removed and is now calculated
at runtime based on start and end addresses.

DynamIQ: Enable MMU without using stack ...

Having an active stack while enabling MMU has shown coherency problems.
This patch builds on top of translation library changes that introduces
MMU-enabling without using stacks.

Previously, with HW_ASSISTED_COHERENCY, data caches were disabled while
enabling MMU only because of active stack. Now that we can enable MMU
without using stack, we can enable both MMU and data caches at the same
time.

NOTE: Since this feature depends on using translation table library v2,
disallow using translation table library v1 with HW_ASSISTED_COHERENCY.

Clarify comments in xlat tables lib and fixes related to the TLB

Add comments about mismatched TCR_ELx and xlat tables ...

When the MMU is enabled and the translation tables are mapped, data
read/writes to the translation tables are made using the attributes
specified in the translation tables themselves. However, the MMU
performs table walks with the attributes specified in TCR_ELx. They are
completely independent, so special care has to be taken to make sure
that they are the same.

This has to be done manually because it is not practical to have a test
in the code. Such a test would need to know the virtual memory region
that contains the translation tables and check that for all of the
tables the attributes match the ones in TCR_ELx. As the tables may not
even be mapped at all, this isn't a test that can be made generic.

The flags used by enable_mmu_xxx() have been moved to the same header
where the functions are.

Also, some comments in the linker scripts related to the translation
tables have been fixed.

Introduce the new BL handover interface ...

This patch introduces a new BL handover interface. It essentially allows
passing 4 arguments between the different BL stages. Effort has been made
so as to be compatible with the previous handover interface. The previous
blx_early_platform_setup() platform API is now deprecated and the new
blx_early_platform_setup2() variant is introduced. The weak compatiblity
implementation for the new API is done in the `plat_bl_common.c` file.
Some of the new arguments in the new API will be reserved for generic
code use when dynamic configuration support is implemented. Otherwise
the other registers are available for platform use.

Fully initialise essential control registers ...

This patch updates the el3_arch_init_common macro so that it fully
initialises essential control registers rather then relying on hardware
to set the reset values.

The context management functions are also updated to fully initialise
the appropriate control registers when initialising the non-secure and
secure context structures and when preparing to leave EL3 for a lower
EL.

This gives better alignement with the ARM ARM which states that software
must initialise RES0 and RES1 fields with 0 / 1.

This patch also corrects the following typos:

"NASCR definitions" -> "NSACR definitions"

PSCI: Build option to enable D-Caches early in warmboot ...

This patch introduces a build option to enable D-cache early on the CPU
after warm boot. This is applicable for platforms which do not require
interconnect programming to enable cache coherency (eg: single cluster
platforms). If this option is enabled, then warm boot path enables
D-caches immediately after enabling MMU.

Enable data caches early with hardware-assisted coherency ...

At present, warm-booted CPUs keep their caches disabled when enabling
MMU, and remains so until they enter coherency later.

On systems with hardware-assisted coherency, for which
HW_ASSISTED_COHERENCY build flag would be enabled, warm-booted CPUs can
have both caches and MMU enabled at once.

Add PMF instrumentation points in TF ...

In order to quantify the overall time spent in the PSCI software
implementation, an initial collection of PMF instrumentation points
has been added.

Instrumentation has been added to the following code paths:

- Entry to PSCI SMC handler.  The timestamp is captured as early
  as possible during the runtime exception and stored in memory
  before entering the PSCI SMC handler.

- Exit from PSCI SMC handler.  The timestamp is captured after
  normal return from the PSCI SMC handler or if a low power state
  was requested it is captured in the bl31 warm boot path before
  return to normal world.

- Entry to low power state.  The timestamp is captured before entry
  to a low power state which implies either standby or power down.
  As these power states are mutually exclusive, only one timestamp
  is defined to describe both.  It is possible to differentiate between
  the two power states using the PSCI STAT interface.

- Exit from low power state.  The timestamp is captured after a standby
  or power up operation has completed.

To calculate the number of cycles spent running code in Trusted Firmware
one can perform the following calculation:

(exit_psci - enter_psci) - (exit_low_pwr - enter_low_pwr).

The resulting number of cycles can be converted to time given the
frequency of the counter.

Introduce PSCI Library Interface ...

This patch introduces the PSCI Library interface. The major changes
introduced are as follows:

* Earlier BL31 was responsible for Architectural initialization during cold
boot via bl31_arch_setup() whereas PSCI was responsible for the same during
warm boot. This functionality is now consolidated by the PSCI library
and it does Architectural initialization via psci_arch_setup() during both
cold and warm boots.

* Earlier the warm boot entry point was always `psci_entrypoint()`. This was
not flexible enough as a library interface. Now PSCI expects the runtime
firmware to provide the entry point via `psci_setup()`. A new function
`bl31_warm_entrypoint` is introduced in BL31 and the previous
`psci_entrypoint()` is deprecated.

* The `smc_helpers.h` is reorganized to separate the SMC Calling Convention
defines from the Trusted Firmware SMC helpers. The former is now in a new
header file `smcc.h` and the SMC helpers are moved to Architecture specific
header.

* The CPU context is used by PSCI for context initialization and
restoration after power down (PSCI Context). It is also used by BL31 for SMC
handling and context management during Normal-Secure world switch (SMC
Context). The `psci_smc_handler()` interface is redefined to not use SMC
helper macros thus enabling to decouple the PSCI context from EL3 runtime
firmware SMC context. This enables PSCI to be integrated with other runtime
firmware using a different SMC context.

NOTE: With this patch the architectural setup done in `bl31_arch_setup()`
is done as part of `psci_setup()` and hence `bl31_platform_setup()` will be
invoked prior to architectural setup. It is highly unlikely that the platform
setup will depend on architectural setup and cause any failure. Please be
be aware of this change in sequence.

Introduce COLD_BOOT_SINGLE_CPU build option ...

This patch introduces a new build option named COLD_BOOT_SINGLE_CPU,
which allows platforms that only release a single CPU out of reset to
slightly optimise their cold boot code, both in terms of code size
and performance.

COLD_BOOT_SINGLE_CPU defaults to 0, which assumes that the platform
may release several CPUs out of reset. In this case, the cold reset
code needs to coordinate all CPUs via the usual primary/secondary
CPU distinction.

If a platform guarantees that only a single CPU will ever be released
out of reset, there is no need to arbitrate execution ; the notion of
primary and secondary CPUs itself no longer exists. Such platforms
may set COLD_BOOT_SINGLE_CPU to 1 in order to compile out the
primary/secondary CPU identification in the cold reset code.

All ARM standard platforms can release several CPUs out of reset
so they use COLD_BOOT_SINGLE_CPU=0. However, on CSS platforms like
Juno, bringing up more than one CPU at reset should only be attempted
when booting an EL3 payload, as it is not fully supported in the
normal boot flow.

For platforms using COLD_BOOT_SINGLE_CPU=1, the following 2 platform
APIs become optional:
  - plat_secondary_cold_boot_setup();
  - plat_is_my_cpu_primary().
The Porting Guide has been updated to reflect that.

User Guide updated as well.

Make generic code work in presence of system caches ...

On the ARMv8 architecture, cache maintenance operations by set/way on the last
level of integrated cache do not affect the system cache. This means that such a
flush or clean operation could result in the data being pushed out to the system
cache rather than main memory. Another CPU could access this data before it
enables its data cache or MMU. Such accesses could be serviced from the main
memory instead of the system cache. If the data in the sysem cache has not yet
been flushed or evicted to main memory then there could be a loss of
coherency. The only mechanism to guarantee that the main memory will be updated
is to use cache maintenance operations to the PoC by MVA(See section D3.4.11
(System level caches) of ARMv8-A Reference Manual (Issue A.g/ARM DDI0487A.G).

This patch removes the reliance of Trusted Firmware on the flush by set/way
operation to ensure visibility of data in the main memory. Cache maintenance
operations by MVA are now used instead. The following are the broad category of
changes:

1. The RW areas of BL2/BL31/BL32 are invalidated by MVA before the C runtime is
   initialised. This ensures that any stale cache lines at any level of cache
   are removed.

2. Updates to global data in runtime firmware (BL31) by the primary CPU are made
   visible to secondary CPUs using a cache clean operation by MVA.

3. Cache maintenance by set/way operations are only used prior to power down.

NOTE: NON-UPSTREAM TRUSTED FIRMWARE CODE SHOULD MAKE EQUIVALENT CHANGES IN
ORDER TO FUNCTION CORRECTLY ON PLATFORMS WITH SUPPORT FOR SYSTEM CACHES.

Introduce PROGRAMMABLE_RESET_ADDRESS build option ...

This patch introduces a new platform build option, called
PROGRAMMABLE_RESET_ADDRESS, which tells whether the platform has
a programmable or fixed reset vector address.

If the reset vector address is fixed then the code relies on the
platform_get_entrypoint() mailbox mechanism to figure out where
it is supposed to jump. On the other hand, if it is programmable
then it is assumed that the platform code will program directly
the right address into the RVBAR register (instead of using the
mailbox redirection) so the mailbox is ignored in this case.

Rationalize reset handling code ...

The attempt to run the CPU reset code as soon as possible after reset
results in highly complex conditional code relating to the
RESET_TO_BL31 option.

This patch relaxes this requirement a little. In the BL1, BL3-1 and
PSCI entrypoints code, the sequence of operations is now as follows:
 1) Detect whether it is a cold or warm boot;
 2) For cold boot, detect whether it is the primary or a secondary
    CPU. This is needed to handle multiple CPUs entering cold reset
    simultaneously;
 3) Run the CPU init code.

This patch also abstracts the EL3 registers initialisation done by
the BL1, BL3-1 and PSCI entrypoints into common code.

This improves code re-use and consolidates the code flows for
different types of systems.

NOTE: THE FUNCTION plat_secondary_cold_boot() IS NOW EXPECTED TO
NEVER RETURN. THIS PATCH FORCES PLATFORM PORTS THAT RELIED ON THE
FORMER RETRY LOOP AT THE CALL SITE TO MODIFY THEIR IMPLEMENTATION.
OTHERWISE, SECONDARY CPUS WILL PANIC.
