treewide: remove references to CREDITS ...

The CREDITS file was removed from barebox in 2015 by commit 6570288f2d97
("Remove the CREDITS file"). Remove references to it from several files.

ARM: at91: remove mach/io.h ...

Remove at91_sys_read() and at91_sys_write() since these are no longer
used. This makes mach/io.h empty so remove that aswell.

Copy dmacon + lcdcon2 to atmel_lcdfb_info to minimize
dependency on the atmel_lcdfb_platform_data.

ARM: move DMA alloc functions to dma.h ...

This better separates the DMA from the MMU functionality.

Also move all drivers that only depends on asm/mmu.h for the alloc
functions over to the common header.

The FSF address has changed; The FSF site says that

Instead of updating it each time the address changes,
just drop it completely treewide.
