treewide: remove references to CREDITS ...

The CREDITS file was removed from barebox in 2015 by commit 6570288f2d97
("Remove the CREDITS file"). Remove references to it from several files.

usb: storage: Drop struct SCSI_cmd_block ...

All the info we need to pass to transport function can be captured in
a struct us_blk_dev and two byte arrays, so having a dedicated struct
with many unused fields doesn't really buy us anything. Drop the
struct and convert the rest of the code to pass needed data
explicitly.

Treewide: remove address of the Free Software Foundation ...

The FSF address has changed in the past. Instead of updating it
each time the address changes, just drop it completely treewide.

USB mass storage device driver initial implementation ...

Implemented an initial version of USB mass storage device driver,
supporting USB mass storage devices with SCSI interface and BBB
protocol.  It implements the ATA interface and registers diskovered
LUNs with the disk driver.

- remove trailing white space, trailing empty lines, C++ comments, etc.
  - split cmd_boot.c (separate cmd_bdinfo.c and cmd_load.c)

* Patches by Kenneth Johansson, 25 Jun 2003:

    (work done mostly by Michal Cendrowski and Joakim Kristiansen)
